Phil Heath Explains How His Current Physique Changed on 200 Milligrams of TRT
Since leaving the stage, bodybuilding legend Phil Heath has overhauled his performance-enhancing approach. He recently opened up about using 200 milligrams of testosterone weekly to maintain his physique at 45 years old.
Unlike past Mr. Olympias, Phil Heath battled new challenges in his prime. He had to navigate his seven-year title reign under immense pressure thanks to the emergence of social media. These were obstacles that his predecessor, Jay Cutler, did not have to deal with.
Heath retains most of the mass he carried on stage, consistently leading to questions about his use of performance-enhancing drugs. In a question-and-answer segment on his YouTube channel, he revealed why a steady regimen of testosterone replacement therapy (TRT) has worked wonders for him.

“One of the great questions that we have is how many milligrams of testosterone are you using now? 200. I’m 246-248 [pounds]. Normally around this time, I would be 275-280 pounds,” Heath shared with his fans.
Phil Heath Discusses Changes to His Physique on Testosterone
Since his third-place finish at Mr. Olympia in 2020, ‘The Gift’ has revamped both his health and diet. Combined with stepping away from ‘different PEDs,’ these changes have given him a younger, fresher look recently.
“The diet has changed. The inflammation is definitely leaving my body much more than what it would be if I was doing different PEDs and stuff like that.”
“As you can see I’m kind of getting leaner in the face, so I’m probably looking a little younger as well.”
How TRT Differs From Steroid Use
Just like any substance, TRT comes with clear pros and cons, setting it apart from traditional anabolic steroid use. Most importantly, TRT is often misunderstood because it is a legal therapy though many do consider it a performance-enhancer.
On an exclusive Generation Iron Podcast, bodybuilding vet Victor Martinez explored this very topic. Testosterone isn’t just for social media stars like Liver King and Larry Wheels—it has real, practical uses, especially as we age and face declining testosterone levels. That drop can trigger a bunch of nasty health issues, like low sex drive, insomnia, shrinking muscles, and depression, to name a few.
In light of these issues, it makes perfect sense that seven-time Mr. O Phil Heath is preserving his health and physique with TRT use. He remains one of the most decorated bodybuilders in the IFBB Pro League, and despite years away from the stage, fans still eagerly await his long-anticipated comeback.
